Employment Insurance Compassionate Care Benefit
On
January 4, 2004, the Government of Canada introduced the new Employment
Insurance Compassionate Care Benefit. This benefit is designed to
help ease the stress faced by
Canadians who must choose between their jobs and caring for a gravely
ill relative.

Medical Certificate Form
completed by a medical doctor if you are asking for compassionate care
benefits. You can return this form by mail or deliver it to your local
Service Canada Centre.
Please note that the fees requested by your doctor to complete this
form are entirely at your own expense.
